Support coordination
At MSWA, our Client is at the heart of everything we do. Our experienced and compassionate team will tailor your NDIS services to your needs, ensuring your plan works for you. Let us support your NDIS journey today.
We recognise that navigating the NDIS can be complicated and challenging, and we’re here to help.
Finding and choosing your provider can be overwhelming and time consuming. Some may prefer to manage their NDIS plan themselves. Others feel that they require support to understand and navigate the NDIS and its process. This is where MSWA’s Support Coordination service can help you.
Our services
Support Coordination helps you build the skills that you need to understand and implement your NDIS plan. It is a funded service within your NDIS plan that provides ongoing assistance to connect you to appropriate services and supports.
MSWA Support Coordinators assist you to optimise your NDIS funding and help give you the information you need to make an informed decision about services – from MSWA and externally.
At MSWA, we have two types of services that can help ensure you receive the best supports for your needs and allow you to have choice and control.
This service is available in-person and via telehealth. If you would like further information, please contact us on 08 9365 4888 or via email at [email protected] or complete the form below.
Working behind the scenes
Your Support Coordinator will arrange face-to-face or virtual meetings with you when needed. However, this is only part of the service that a Support Coordinator provides. The majority of their work is done outside these sessions. This involves contacting your service providers, reading and writing reports, communicating with colleagues/service providers/NDIS, answering any questions you have, conducting research and conducting administrative tasks.
Support Coordinators are not funded to provide the following: transport, plan management/case management, rostering of support staff and advocacy.
