Nursing for people living with other Neurological Conditions

MSWA has a team of experienced Community Neurological Nurses who provide individual assessment, care planning and advice for people living with other neurological conditions accessing our block funded programs, residential respite, in-home supports or with individual funding through the NDIS or Disability Services Commission.

MSWA Community Neurological Nurses are committed to providing accurate and relevant information to support your decision-making and care. We currently have experienced nurses working with people with rapidly degenerative conditions, including MND and brain tumours, who provide ongoing support and in-home follow up as part of our block funded programs or the NDIS.  Our nurses are also able to liaise with the hospital and community based health teams to maximise support.

An individualised assessment is usually required to assist with the development of tailored in-home or residential respite Care Plans supporting the individual’s outcomes, which guide our support worker staff delivering your services.

The nurse may also recommend a referral to allied heath staff under one of our funded programs, for assessment and advice regarding aids and equipment, counselling or speech therapy, for example.
