Neurodegenerative Conditions Coordinated Care Program

The Neurodegenerative Conditions Coordinated Care Program (NCCCP) is a new pilot program providing services to assist people to remain in their own homes with support.

The NCCCP seeks to provide timely support for people with rapidly degenerating neurological conditions and whose support needs will increase over time.

Referrals for this program may be received from Support Groups, Service Providers, Disability Services Commission, Neurologists, General Practitioners and Self Referral. People referred for NCCCP services must be eligible for Disability Services Commission funded services, and meet the specific program parameters.
